Click here to downloadThis Microsoft Excel spreadsheet was created in the summer of 2014 and lists practitioners, organizations and support groups along the bottom tabs. The info in the database was accurate at that time, but please contact the resource to make sure what you are seeing is current.
If you want to search a specific geographic area or whether a provider takes your insurance, access the “Home” tab and click on “Find and Select.” Type in what you want to search, and the database will search for you. You need to have Excel on your computer to be able to access the database.
