You may see a variety of health care providers—specialists, optometrists, dentists, allergists, and others, or you may visit an Emergency Room and your medications may have changed.

We won’t know that unless you bring in your medications and we check them with the list we have.

All medicines have side effects. You may have noticed one or more side effect(s) and decided on your own (or from someone’s advice) to stop or cut back on a medicine.

We need to know about the side effect and also if you changed any medications.

That’s why we ask you EVERY TIME you are here. Please bring in ALL of your medicines, including the over-the-counter ones like vitamins or pain relievers.
